Job summary

St. David's Medical Center is seeking a Case Management Assistant (CMA) to support discharge planning and post-acute placement coordination.

You will work under the RN Case Manager or Social Worker to identify barriers and provide administrative support for Case Management functions. Responsibilities include coordinating with care teams, updating on progress, and facilitating referrals to post-acute providers.

Qualifications

• Medical knowledge and terminology.
• Minimum of one year experience in a health care setting.

Responsibilities

• Coordinate with RN Case Manager and Social Worker for support needed.
• Communicate updates on progress and refer questions to the Case Manager/Social Worker.
• Provide clerical support in multidisciplinary rounds and update NATE Tempo as directed.
• Make referrals to post-acute care providers via web-based referral system.
• Monitor PAC referrals and intervene to expedite discharge.
• Prioritize workload to ensure timely contact with PAC providers.
• Communicate PAC status, bed availability and other unresolved barriers to the Case Manager.
