Job summary

Memorial Health University Medical Center in Savannah, GA, seeks a Case Manager (MSW) to perform biopsychosocial assessments, crisis intervention, and non-clinical care coordination across inpatient floors and the ED.

You will collaborate with nurses, physicians, and payers to address barriers to discharge, develop patient-centered discharge plans, and connect patients and families with resources and emotional support throughout the care continuum.

Qualifications

• MSW degree required.
• LMSW/LCSW licensure preferred.
• 5 years of hospital patient care experience preferred.
• 3 years of hospital social work experience required.

Responsibilities

• Collaborate with RNs, case managers, physicians and payers to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid and private payer regulations and processes; understand length-of-stay management.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between care teams and patients/families/caregivers; support patient and family decisions.
• Provide emotional support and crisis intervention for patients and families during end-of-life or complex situations.
