Job summary

HCA Florida Westside Hospital is seeking a Case Management Assistant to support discharge planning and post-acute placement. You will coordinate with the RN Case Manager and Social Worker, prepare PAC referrals, and liaise with payors to obtain authorizations where applicable.

The role requires medical knowledge, terminology, and a minimum of two years in a health care setting. CNA/EMT licensing is preferred. Join our team supporting patient care and efficient transitions.

Qualifications

• Associate degree preferred.
• Medical knowledge and terminology required.
• Minimum of two years experience in a health care setting required.
• Currently licensed/certified as a health care professional (CNA, EMT) preferred.

Responsibilities

• Coordinate with RN Case Manager and Social Worker for discharge planning and PAC referrals.
• Provide clerical support for multidisciplinary rounds and update case work notes.
• Make referrals through PAC systems and coordinate post-acute care placements.
• Monitor PAC referrals and intervenes to expedite placement with facilities.
• Obtain physician signatures on required case management documents.
