Job summary

Baptist Health in Jacksonville, FL, is seeking a Care Coordination Assistant for the Baptist Metro campus. This is a full-time, day shift role that provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help coordinate patient-centered care.

You will assist with discharge planning, coordinate meetings and referrals, and ensure accurate documentation in the electronic medical record, supporting smooth transitions for patients and families.

Qualifications

• 1-2 years of hospital experience.
• High School Diploma required.
• Basic Life Support (BLS) certification.
• Driver's License.

Responsibilities

• Provide non-clinical and clerical support to Care Coordination and Pediatric Social Work teams.
• Assist with coordination of care and discharge planning activities.
• Support patient and family care transitions across the continuum of care.
• Communicate with internal departments and external agencies as directed.
• Maintain accurate documentation and assist with data entry in the electronic medical record.

• Ensure timely completion of assigned administrative tasks.

Under the direction of a licensed professional, the Care Coordination Assistant provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care. This role plays an important part in supporting smooth care transitions for patients and families.

Care Coordination Assistant, Job Responsibilities:
