Job summary

Otterbein SeniorLife is seeking a Care Coordination Specialist to work with Admissions Specialists, HealthCare Navigators, and community staff to ensure residents and families receive outstanding service from arrival through stay.

You will lead tours, develop referral networks, and oversee case management and discharge planning, while supporting business office functions and insurance appeals as needed.

Qualifications

• Graduate of approved nursing program.
• Valid Ohio LPN license and CPR certification.
• Valid driver’s license.
• Minimum of three years in customer service or sales; healthcare/long-term care experience preferred.
• Computer skills including Word and Excel.

Responsibilities

• Provide exceptional customer service to incoming residents and families.
• Collaborate with intake specialists and community teams to facilitate admissions.
• Develop relationships with referral partners to grow patient referrals.
• Oversee admissions flow and bed readiness for new residents.
• Coordinate discharge planning and insurance appeals when needed.
• Support billing and business office functions.
