The role emphasizes accurate data entry, timely processing, and professional liaison to care teams. You will handle referrals, maintain logs, and ensure compliance with Florida regulations while supporting a compassionate patient experience.

Qualifikationen

• Minimum of 1 year of experience in a healthcare setting.
• CNA experience preferred.
• Clinical sales experience preferred.
• Maintains up-to-date skill sets as required to care for patients.
• Proficient use and entering information into Electronic Medical Records preferred.

Aufgaben

• Ensures accurate and timely entry of patient referrals in all business lines.
• Receives all incoming referrals and requests supporting documentation to facilitate the referral-to admission process.
• Initiates, updates, and completes assigned patient referrals by documenting patient information in the electronic health record.
• Referrals to be sent to Care Coordinator or covering staff within 15 minutes of referral receipt.
• Ensures consents are accurate and completed according to standards and regulations.
• Maintains close contact with Admission Nurses and Hospital Nurse Care Coordinators, informing of all new referrals and progress of existing referrals.
