Qualifications

Responsibilities

• Identifies current and potential complex problems and plans appropriate interventions.
• Collaborates with leaders to develop standards of care.
• Documents patient information integrating assessment, pathophysiology, and regimen.
• Acts as patient advocate and communicates concerns to the health care team.
• Coordinates and delivers care to ensure safety, quality, and positive patient experience.
