Saint Joseph’s Hospital and Medical Center in Phoenix, AZ is seeking a Cardiovascular Program Specialist to coordinate referrals, support staff and patients, and ensure timely access to cardiovascular evaluation and procedures. The role emphasizes clinical expertise in end-stage cardiovascular care, strong communication, and autonomous judgment.

The position requires RN licensure, 5+ years cardiovascular experience, and a track record in high-level care coordination within a hospital setting.

Competenze

• RN with cardiovascular experience required or equivalent work experience.
• Evidence in caring for acute and chronic heart and vascular patients.
• Registered Nurse: AZ licensure or eligible.

Mansioni

• Coordinate the referral process and introduction of patients for evaluation.
• Receive referrals from physicians and hospitals and facilitate rapid access to the system.
• Collaborate with Medical Director and multidisciplinary team to schedule evaluation and procedures.
• Triages referrals to ensure patient needs are met on a priority basis.
• Communicate plans for acceptance to referring providers and hospitals.
