Job summary

HonorHealth in Phoenix seeks an experienced Registered Nurse for Cardiovascular Navigation Services. The day shift works across the Deer Valley and John C. Lincoln campuses, collaborating with vascular surgeons and APPs to coordinate care for vascular patients.

Requirements include an RN license, BCLS, 1 year of inpatient/outpatient experience, and 2 years nursing experience. ACLS preferred; sign-on bonus up to $10,000.

Qualifications

• RN required with 1 year inpatient/outpatient experience.
• 2 years nursing experience required.
• 3 years experience in designated specialty preferred.
• BCLS certification required; ACLS preferred.
• RN license (state licensure or compact) required.

Responsibilities

• Collaborates with patients, families, and caregivers to coordinate care across the continuum.
• Develops standards of care and coordinates referrals and consultations.
• Ensures timely tests are performed, results reviewed with physician, and patient notified.
• Influences patient outcomes and satisfaction by efficient resource use.
• Documents information in the medical record and maintains HIPAA compliance.
• Performs assessment, data entry, scheduling and related duties.
