Qualifications

• 5 years of cardiovascular experience upon hire.
• Experience in caring for acute and chronic heart and vascular patients.

Responsibilities

• Coordinates the referral process and introduction of patients for evaluation.
• Receives and responds to incoming referrals from physicians and hospitals.
• Works with the medical director to schedule evaluations and procedures.
• Communicates plans back to referring physician and/or hospital.
• Educates the patient and family on the referral process.
• On-call nights and weekends as required.
• Performs chart reviews and data submission for performance indicators.
• Develops policies and procedures to improve care quality.
