Job summary

ProMedica Monroe Physicians is seeking an RN Program Coordinator in the Cardiology department to manage care from hospital discharge through follow-up visits. You will coordinate the care plan, educate patients on procedures and tests ordered by providers, and review health risk assessments with the medical team.

You will monitor patient charts for test results, discuss findings with providers and patients, and handle preoperative clearance with physicians as needed.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.

Responsibilities

• Coordinate patient care from hospital discharge through follow-ups.
• Educate patients on procedures and testing orders.
• Review health risk assessments and adjust plans with providers.
• Review preoperative surgical clearance requests with providers.
• Triage patient calls and provide health referrals as appropriate.
