Job summary

The RN Program Coordinator at ProMedica is the point person for ensuring collaborative care of patients from hospital discharge through all follow-ups. In their coordination, RN Program Coordinators facilitate the patient plan of care, provide education and instructions for procedures and/or testing ordered by providers, and review the patient’s Health Risk Assessment.

The RN Program Coordinator monitors patients’ charts for test results and reviews them with the provider and the patient.

Qualifications

• Current State license as a Registered Nurse (RN).
• One year of relatable or equivalent experience.
• ACLS and BLS certification.

Responsibilities

• Coordinate patient care from discharge through follow-ups, ensuring collaborative care.
• Explain procedures and testing ordered by providers and review Health Risk Assessments.
• Monitor patient charts for test results and discuss with providers and patients; manage preoperative clearances as needed.
