Job summary

ProMedica in Toledo is seeking an RN Program Coordinator for the Cardiology department. The role centers on coordinating patient care from hospital discharge through follow-ups, educating patients, and reviewing health risk assessments.

You will monitor test results, manage preoperative clearances with providers, and may triage calls with health referrals as needed. This is a part-time, dayshift position in the United States.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.
• EPIC experience.
• Bachelor of Science in Nursing preferred.
• 2+ years relatable experience.

Responsibilities

• Coordinate collaborative post-discharge patient care through follow-ups.
• Educate patients and instruct procedures/tests ordered by providers.
• Review health risk assessments with providers and patients.
• Monitor charts for test results and discuss them with clinicians.
• Triage patient calls and provide health advice/referrals.
