Job summary

ProMedica Northwest Ohio Cardiology Consultants is seeking an RN Program Coordinator to serve as the point person for coordinating care from hospital discharge through all follow-ups. You will facilitate the patient plan of care, provide education for procedures and testing ordered by providers, and review Health Risk Assessments with patients.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.
• PREFERRED: Bachelor of Science in Nursing.
• EPIC experience.
• 2+ years relatable experience.

Responsibilities

• Coordinate patient care from hospital discharge through all follow-ups.
• Educate and instruct for procedures and testing ordered by providers.
• Review the patient’s Health Risk Assessment.
• Monitor patients’ charts for test results and review them with the provider and patient.
• Review and manage preoperative surgical clearance requests with providers.
• Triage patient calls and provide health advice and referrals as appropriate.
