Job summary

ProMedica, at Ebeid Children's Hospital, seeks an RN Program Coordinator to ensure coordinated care from hospital discharge through follow-ups. You will manage the patient plan of care, provide procedure instructions, and review Health Risk Assessments with the care team.

You will monitor charts for test results, review preoperative clearances with providers, and triage patient calls, offering health advice and referrals as appropriate.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.

Responsibilities

• Coordinate patient care plan from discharge through follow-ups.
• Provide education and instructions for procedures or testing ordered by providers.
• Review Health Risk Assessments and monitor test results with providers.
• Review preoperative surgical clearance requests with providers.
• Triage patient calls and offer health advice and referrals as appropriate.
