Job summary

Confluence Health in Washington state is seeking a Patient Referral Coordinator to coordinate new patient referrals, track them, schedule appointments and close the loop on referrals for hospital and ancillary services.

You will communicate with clinical staff, providers, payors and patients, using Epic to align physician availability with patient preferences while maintaining confidentiality and delivering strong customer service.

Qualifications

• High school diploma or equivalent.
• Ability to prioritize, manage time, and work independently.
• Excellent customer service skills and attention to detail.

Responsibilities

• Manage referrals promptly with emphasis on urgent cases.
• Coordinate documentation with external agencies.
• Use Epic to match physician availability with patient preferences.
• Make outbound calls to patients for scheduling.
• Contact provider offices for referrals/orders inquiries.
• Interact with staff at all levels in a fast-paced, professional manner.
• Handle multiple tasks with interruptions and confidentiality.
• Maintain confidential records per policies and RCW.
• Balance multiple priorities in a fast-paced environment.
• Demonstrate problem-solving and critical thinking.
• Communicate effectively and tactfully.
• Maintain organization and follow-through.
• Participate in process improvement related to patient flow.
