The GW Medical Faculty Associates in Washington, DC is seeking a Nurse Navigator to provide proficient nursing care for individuals or groups with less predictable problems and to lead in implementing high-quality nursing care.

The role requires assessing patient baselines, developing and evaluating nursing plans, collaborating with physicians, and providing tailored patient education to maximize participation in care.2100

Qualifications

• Associate’s degree in nursing or an equivalent combination of training and experience.
• Current D.C . Licensure as a Registered Nurse or evidence of application for licensure in D.C. and current out-of-state is necessary.

Responsabilités

• Uses advanced nursing knowledge and the nursing process to assess the patient’s baseline including past and present physical, psychosocial, and cultural factors.
• Define a nursing problem list based on assessment and medical diagnosis.
• Develop a nursing plan aimed at optimal resolution of nursing diagnoses.
• Evaluate and document patient progress toward goals.
• Collaborate with healthcare professionals to implement a comprehensive plan of care.
• Provide an individualized nursing care plan to maximize patient participation in care.
• Collaborate with a physician to revise the nursing plan and goals as needed.
• Implement patient education programs to meet healthcare needs.
• Assist physicians with day-to-day patient care: taking histories, assessing visit reasons, and supporting procedures and treatments.
