Qualifications

• PharmD degree and active Texas license.
• Multi-state licensure required.
• Minimum 3–5 years of clinical cardiology experience.
• Experience with GDMT and titration.

Responsibilities

• Review cardiology med profiles for GDMT optimization.
• Recommend initiation, titration, and optimization of therapies.
• Conduct telephonic patient reviews and education.
• Collaborate with RN Care Managers and cardiology providers.
• Document recommendations promptly in the EMR.
