Qualifications

• Graduate of accredited Nursing program.

Responsibilities

• Facilitates coordination of care across settings for a designated population, ensuring labs and referrals are completed.
• Performs patient care and procedures with sound clinical judgment and approved policies.
• Cleans and sterilizes instruments and maintains supplies.
• Assesses patient readiness to learn and provides current education materials.
• Intervenes to prevent risks based on observation of clinical factors.
• Triages calls to appropriate level of care and provides healthcare guidance.
