Job summary

Carle Health is seeking a Licensed Practical Nurse to join the nursing team in Portland, OR. The LPN will support RNs with high-acuity patients and perform tasks including med passes, IV piggybacks, wound care, and care plan documentation after the initial RN assessment.

The role emphasizes collaboration with RNs/APPs, patient education, and ensuring safe care transitions, with documentation and discharge instructions reinforced by the LPN.

Qualifications

• Licensure as LPN (IDFPR).
• BLS within 30 days required by AHA.
• College diploma: Licensed Practical Nurse.

Responsibilities

• Passes all medications, hangs IV piggybacks and fluids, excluding IV Push meds.
• Charts assessments on assigned patients after RN initial assessment.
• Charts on care plans opened by the RN; assists with new care plans as needed.
• Coordinates with physicians/APPs to receive orders and coordinate care.
• Complete dressing changes, trach care, NG tube care and feedings.
• Care for ostomy; reinforce education and discharge instructions.
