Job summary

Foundation Health Partners in Fairbanks, AK is seeking a dedicated Licensed Practical Nurse (LPN) for the Denali Center care team. You will provide hands-on nursing care, support diagnostic procedures, and collaborate with physicians to deliver patient-centered care for adults, including seniors.

This role includes a compensation range of $25.88–$40.44 per hour, with a $5,000 retention bonus, shift differentials, and benefits such as medical, vision, dental, and 401k with employer match.

Qualifications

• Must possess a current, valid LPN or LVN license in state of practice.
• BLS certification is required; additional certification or continuing education may be required; IV therapy program may be required in certain facilities.

Responsibilities

• Observes, compiles and reports the patient's physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Contributes to plan of care under direction of the registered nurse, including the discharge plan, utilizing assessment data and input from patient, family and health team.
• Implements care based on delegated interventions, physician orders and nursing plan of care, within scope of practice; may supervise non-licensed personnel.
• Reports condition of patient and progress toward goals to the RN/ physician; documents patient response and teaching effectiveness.
• Documents observation, implementation and data in the patient record; ensures accurate physician orders and timely entries.
• May coordinate admission processes and communicate with residents, families and referral sources regarding admissions; uphold policies and patient-centered service standards.
