Foundation Health Partners seeks a compassionate Licensed Practical Nurse (LPN) to join our patient-centered team in Alaska. You will provide direct patient care and clinical support under licensed providers to ensure safe, high-quality care.

The role includes collaboration with RN and physicians, documentation, and participation in care planning and discharge processes. Requires Alaska licensure and BLS; opportunities for education benefits and growth are offered.

Qualifications

• Must possess a current Alaska RN license.
• BLS certification is required.
• Additional ACLS, PALS, or continuing education may be required depending on area of practice.

Responsibilities

• Provide nursing care under the supervision of a registered nurse and licensed physician.
• Contribute to the plan of care and document patient care in accordance with policies and standards.
• Communicate patient status and progress to the care team and document accurately.
• Coordinate admission and discharge planning as part of the care process.
• Assist with data collection, assessment, and delegation of tasks within scope of practice.
