Job summary

Qualifications

• Must possess a current, valid LPN or LVN license in state of practice.
• BLS certification is required.

Responsibilities

• Observes, compiles and reports patient condition and progress to the RN or physician.
• Contributes to plan of care and discharge planning with input from patient, family and health team.
• Implements care per physician orders and nursing plan of care; may supervise non-licensed personnel.
• Maintains accurate documentation of observations and interventions in patient records.
• Coordinates admission and transition processes with residents, families and referrals; follows regulatory requirements.
