Job summary

Qualifications

• Graduate of accredited school of nursing.
• Proficient in English with strong verbal and written communication; proficient computer skills.

Responsibilities

• Clinical practice and care management for assigned patients.
• Performs psychosocial/physiologic assessments and updates care plans.
• Educates patients and families and coordinates discharge planning.
• Collaborates with the healthcare team to ensure safe, efficient care.
• Documents all patient care activities per documentation standards.
