Responsibilities

• Conduct comprehensive CANS assessments for children, adolescents, and transitional-age youth.
• Coordinate with families, providers, and community partners to connect services.
• Maintain accurate EHR and CANS portal documentation per requirements.
• Collaborate with clinical staff to translate assessment findings and next steps.
• Travel within designated service areas.
• Ensure HIPAA compliance and confidentiality.
