Integrated Services for Behavioral Health (ISBH) is seeking a CANS Assessment Associate based in southeastern Ohio to conduct comprehensive CANS assessments for children, adolescents, and transitional‑age youth and to coordinate with families, providers, and community partners.

The role involves documenting findings in the EHR/CANS portal, ensuring HIPAA compliance, traveling within designated service areas, and facilitating timely access to care across OhioRISE pathways and related programs.

Qualifikationen

• High school diploma or GED required.
• Current Ohio CANS certification or ability to certify within the first 21 days of employment.
• 2–3 years of experience in community mental health or related human services setting.
• Knowledge of Medicaid regulations and compliance requirements.
• Access to reliable transportation and valid driver's license.

Aufgaben

• Conduct comprehensive CANS assessments for children, adolescents, and transitional‑age youth (birth through age 21) in virtual, home, school, and community settings.
• Build knowledge of OhioRISE referral pathways and program offerings including 1915C waivers, YFC/Ifast/IHBT, and QRTP requests.
• Maintain accurate, timely records in the EHR and CANS portal per agency and state requirements.
• Collaborate with clinical staff, families, and community partners to translate assessment findings and next steps.
• Travel within designated service areas and maintain compliance with HIPAA and professional guidelines.
