Job summary

Beth Israel Lahey Health is seeking a Nurse Navigator to coordinate ambulatory oncology care across BIDMC cancer centers. Qualifications

• Bachelor's degree required.
• Registered Nurse license required.
• Basic Life Support (BLS) certification required.
• 3–5 years related work experience in clinical medical-surgical settings.
• Strong analytic skills and data analysis experience.
• Excellent interpersonal and collaboration skills.
• Experience with interdisciplinary health care approaches.
• Experience with computer systems including MS Office and web-based applications.

Responsibilities

• Initiates communication with patients after positive findings and guides them through the health care system.
• Acts as liaison among patient, PCP, surgeon, medical oncologist and other providers.
• Provides health education and maintains patient learning materials.
• Identifies care department personnel and builds relationships for coordinated care.
• Assists with questions regarding payment and arranges patient transportation as needed.
• Tracks interventions and participates in medication reconciliation during transitions of care.
