The role coordinates care with physicians and specialists, educates patients and families, and supports access to services, transportation, and social supports while maintaining high-quality, patient-centered care across departments.

Qualifications

• Bachelor's degree is required.
• RN license required.
• American Heart Association – BLS certificate required.
• 3–5 years related clinical medical-surgical experience.

Responsibilities

• Initiates communication with patients upon learning they have a positive finding and guides them through the health care system.
• Provides health education and maintains a learning library for patients (language-specific materials).
• Identifies personnel in departments involved with their care and develops relationships with them.
• Assists patients with questions/ concerns regarding payment of services and arranging transportation when needed.
• Tracks interventions and outcomes and participates in medication reconciliation during transitions of care.
