Qualifications

Responsibilities

• Coordinate care across the continuum, guiding patients and families through diagnosis, treatment and survivorship.
• Educate patients and families about illness, treatment options and preventative measures.
• Collaborate with breast surgeons, referring providers and multidisciplinary care teams to facilitate timely access to services.
• Support quality and outcomes initiatives, clinical pathway development, survivorship planning, and accreditation standards.
• Maintain professional relationships with nurses, physicians and colleagues, supporting regulatory compliance and patient safety.
