Job summary

The Cancer Center at GRMC is seeking a Care Navigator to act as a patient advocate and coordinator for the outpatient cancer center. The role autonomously coordinates, negotiates, procures services and resources, and manages care for complex patients to support quality and cost-efficient outcomes.

You will guide patients through the continuum of cancer care within and outside GRMC, providing individualized support and education to patients and families.

Qualifications

• BSN required; Master’s preferred.
• 2–5 years of acute care RN or nurse case management experience.
• 3+ years oncology clinical experience preferred.
• ACLS certification preferred.
• OCN or ONN-CG certification preferred.
• Case management or managed care nursing certification preferred.

Responsibilities

• Act as patient advocate and care coordinator for outpatient cancer center.
• Coordinate, procure services and resources for complex patients.
• Guide patients through the continuum of cancer care inside and outside GRMC.
• Provide individualized support and education to patients and families.
• Address barriers to timely access and enhance patient experience.
