Job summary

Novant Health is seeking a Clinical Navigator to serve as the primary liaison between patients and their multidisciplinary medical team, providing advocacy, education, care coordination, and ongoing support throughout the cancer treatment journey.

You will coordinate care across the oncology disciplines—medical, radiation, surgery, nursing, radiology, pathology—and facilitate timely communication among all members of the patient’s care team.

Qualifications

• High School or GED required; 4 Year / Bachelors Degree preferred.
• Three to five years of nursing experience required.
• Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.
• Ability to successfully complete generic and department-specific skills validation and competency testing.

Responsibilities

• Serve as a consistent point of contact and trusted resource for patients and families throughout the cancer care continuum.
• Coordinate care across the multidisciplinary team, including medical oncology, radiation oncology, surgery, nursing, radiology, pathology, and other specialty services.
• Facilitate timely communication and collaboration among members of the patient’s care team.
