Job summary

Qualifications

Responsibilities

• Completes biopsychosocial assessment of patients.
• Formulates diagnostic summary evaluating clinical needs and conditions.
• Formulates individualized treatment plans based on diagnostic summary.
• Coordinates team input, family and patient goals in treatment plans; include family when appropriate.
• Continuously revises treatment plans based on progress.
• Introduces patient and family to 12-step recovery.
• Prepares achievable discharge plans with patient, team, and family.
• Facilitates positive changes in attitudes, feelings, thoughts, behaviors, and decision making.
