Advocate Health in Illinois seeks a dedicated addiction case manager to assess patients and families, develop biopsycho-social summaries, and create individualized treatment plans with the care team. Coordinate input, revise plans as needed, and introduce patients to 12-step recovery.

You will lead group counseling, supervise students, deliver education on addiction and recovery, participate in quality improvement activities, and collaborate on program development to enhance patient care.

Qualifications

• Associates Degree in Behavioral Health or equivalent; Bachelors preferred.
• 2 years of experience in addiction or behavioral health field.
• Ability to work with diverse age groups and families.

Responsabilités

• Case manage addiction patients and families; complete biopsychosocial assessments and diagnostic summaries.
• Develop individualized treatment plans based on clinical needs identified in the assessments.
• Coordinate input from the team, family, and patient to formulate treatment plans; revise as progress dictates.
• Introduce patient and family to 12-step recovery and prepare discharge plans with the care team.
• Conduct addiction-focused group counseling using peer-centered approaches.
• Provide clinical supervision to students and supervisees; support education and training.
• Deliver education on addiction and recovery; present information clearly and respectfully.
• Participate in quality improvement activities and program development.
