Job summary

Qualifications

• BSN required
• Current State of Illinois Registered Professional Nurse (RN)
• Basic Life Support (BLS) certification
• Strong interpersonal skills and ability to collaborate with others
• ADN acceptable with commitment to obtain a BSN within three years of first day of employment

Responsibilities

• Perform comprehensive patient assessments with documented outcomes.
• Communicate changes in patient condition promptly to the care team.
• Derive realistic goals from nursing diagnoses and update the plan of care accordingly.
• Coordinate discharge planning and referrals as needed.
• Educate patients and families using teach-back and provide portal and resource information.
• Maintain safe environment and participate in unit-based quality initiatives.
