Northwestern Medicine is seeking a Registered Nurse with a BSN to provide high-quality patient care under the Relationship Based Care model. The position requires adherence to nursing standards, ethics, and regulatory requirements while coordinating with a multidisciplinary team.

Responsibilities include comprehensive patient assessments, timely communication of condition changes, and active participation in discharge planning and education to patients and families.

Competenze

• BSN required and Illinois RN license required.
• BLS certification required.
• ADN acceptable with commitment to obtain BSN within three years of employment.

• Performs a comprehensive assessment on all assigned patients with documented outcomes.
• Communicates changes in patient condition promptly to the care team.
• Develops and documents realistic goals on the plan of care and revises as needed.
• Collaborates with patient, family, and health care team for discharge planning and referrals.
• Demonstrates safe medication administration and unit-based safety practices.
• Leads and participates in unit huddles and quality improvement initiatives.
