Qualifications

• BSN required and Illinois RN license in good standing.
• BSN required; ADN with commitment to obtain BSN within 3 years acceptable.
• Current IL RN license and Basic Life Support certification.
• Strong interpersonal skills and collaboration ability.
• Analytical skills for data-driven patient care.

Responsibilities

• Performs a comprehensive assessment on all assigned patients with documented results.
• Communicates changes in patient condition promptly to the care team.
• Uses nursing diagnoses and assessment data to set measurable plan-of-care goals.
• Prioritizes patient needs and documents nursing diagnoses accordingly.
• Collaborates with patient, family, and care team for planning and discharge referrals.
• Evaluates patient responses to interventions and adjusts the plan as needed.
• Ensures discharge planning, medication reconciliation, and patient education.
