Job summary

• Performs comprehensive assessment on all assigned patients and documents it.
• Communicates changes in patient condition promptly.
• Collaborates with patient, family and care team to plan care.
• Evaluates patient responses to interventions and updates care plan.
• Supports discharge planning and referrals as needed.
• Ensures safe medication administration and promotes a culture of safety.
