Qualifications

Bachelor of Science in Nursing (BSN).
Current State of Illinois Registered Professional Nurse license.
American Heart Association Basic Life Support (BLS).
Strong interpersonal skills and ability to collaborate with others.
Strong analytic abilities.
ADN acceptable with commitment to obtain a BSN within three years of employment.

Responsibilities

Assess and record patient status at admission and point of care.
Organize health data for accurate, accessible records.
Identify problems needing immediate attention and validate diagnoses with the team.
Document diagnoses and plan of care to support outcome achievement.
Initiate discharge planning early and discuss plans with patient/family.
Provide patient education based on evidence-based practice.
Communicate and collaborate with the care team to coordinate patient care.
