Educational Requirements: Bachelor's Degree Required with relevant experience.
Qualifications and Experience: Experience working with mental health, crisis intervention, and managing trauma-triggered behavior. Excellent time management skills; stress-resistant, flexible; team player; open to feedback; creative; exceptional organizational skills; willingness to perform CPR/First Aid/AED/CPI/Naloxone. Preferred knowledge of AWARDS.
Hours: Full-Time, 35 hours per week, Monday-Friday Schedule.
Pay/Salary: $50,000 annually
Under the supervision of the Program Coordinator, the Case Manager provides supportive services to formerly homeless adults living in scattered-site apartments throughout the Bronx. The population served includes individuals diagnosed with serious mental illness, seniors, veterans, medically frail individuals, and those with a forensic history. The Case Manager meets individually with clients in their apartments, completes required documentation promptly and in accordance with agency policy, and regularly conducts fieldwork.
