ACMH, Inc. in The Bronx is hiring a Housing Specialist for the Assertive Community Treatment (ACT) program. You will lead integration of housing goals with clinical care and assist clients in accessing housing of their choice, including completing HRA 2010e applications.

The role reports to the Team Leader, ACT, with a schedule of Monday–Friday, 9:00am–5:00pm. Strong communication and documentation skills are essential.

Formación

• B.A. or M.A. in social services or related field and one year of direct service experience.
• Demonstrated competency in clinical documentation and completing housing applications.
• Strong written and verbal communication skills.

Responsabilidades

• Develop rapport with clients to engage them in improving health and wellness.
• Administer standardized health and psychosocial risk screenings per program protocols.
• Collaborate with care team to identify needs and develop care plans for optimal health outcomes.
• Integrate housing goals with team tasks and assist with HRA housing applications.
• Schedule housing interviews and tours and escort clients to appointments.
• Collaborate with community housing providers and landlords.
• Document all interventions in the EHR and update care plans as needed.
• Provide transitional care following hospitalization and participate in team meetings.
