Job summary

ACMH, Inc. in The Bronx is seeking a Senior Case Manager/Case Manager for the Bronx Treatment Apartment (BTA).

The role provides case coordination and delivers community rehabilitation services, reporting to the Program Director, on a 9:00am-5:00pm, Monday–Friday schedule. Responsibilities include home visits, psychosocial assessments, coordinating psychiatric and housing services, developing individualized plans, crisis intervention, and thorough documentation.

Qualifications

• MA in psychology or human service field; BA with 3 years direct care experience.
• Experience with adults with serious mental illness preferred.
• Strong written and verbal communication skills.

Responsibilities

• Conducts home visits for assigned case load.
• Assures access to psychiatric, substance use, medical services, and entitlements.
• Monitors client medication, including adherence and side-effects.
• Develops an individualized Rehabilitation Plan (Service/Support Plan).
• Delivers community rehabilitation services and housing support services.
• Documents client progress and updates the Service/Support Plan.
• Trains clients in safety procedures and conducts fire drills and drills.
• Prepares residents for transition to independent living and makes referrals.
• Facilitates crisis intervention when needed.
• Maintains client charts and submits notes and reports.
