ACMH in The Bronx is seeking a Housing Specialist for Assertive Community Treatment (ACT) to integrate housing goals with multidisciplinary care. The role includes assisting clients with accessing housing, completing HRA 2010e applications, securing benefits, and coordinating with the care team to ensure continuity of care within CTI protocols.

The position reports to the Team Leader, ACT, and operates Monday–Friday, 9:00am–5:00pm in the Bronx.

Qualifications

• B.A. or M.A. in social services or related field with at least one year of direct service experience.

Responsabilités

• Engage clients to improve health and wellness.
• Administer standardized health and psychosocial risk screenings.
• Develop a comprehensive care plan from screenings.
• Coordinate with care team to meet housing and health needs.
• Facilitate 2010e housing applications and housing interviews.
