Compensation: $21/hour
Schedule: Monday-Friday
Location: Wasilla, AK

As a Bridge Care Coordinator, you will be an essential part of our team, providing support and companionship to aging patients through our Bridge Program. Your responsibilities will include:
- Visiting patients in their homes and providing peace of mind.
- Educating families about available healthcare services.
- Connecting patients with community resources.
- Helping transition patients between care services, including hospice.
- 18+ years old
- Healthcare experience required
- Strong organizational & computer skills
- Ability to work independently
- Valid driver's license & clean driving record
- Reliable transportation & current auto insurance (MVR check required)
- Negative TB test
