Ancora Home Health & Hospice in Anchorage, AK is seeking a compassionate Bridge Care Coordinator to support aging patients through our Bridge Program. You will visit patients at home, educate families, and help transition between care services, including hospice, providing peace of mind and practical support.

Your Role as a Bridge Care Coordinator

As a Bridge Care Coordinator, you will be an essential part of our team, providing support and companionship to aging patients through our Bridge Program. Your responsibilities will include:
