Qualifications

• Current Registered Nurse (RN) license in Alaska.
• Home Health / Hospice experience as a Registered Nurse preferred.
• Ability to utilize full range of movement to transfer patients.
• Effective verbal & written communication & time management skills.
• Valid Driver's License (clean record) & reliable transportation.
• Proof of current auto insurance & registration.
• Valid AK Fingerprint Clearance Card (we can help).
• Current CPR Certification (we can help) & Negative TB Test (we can test).

Responsibilities

• Deliver exceptional home health and end-of-life care to patients in their homes.
• Coordinate with the Interdisciplinary Group to develop comprehensive care strategies.
• Maintain meticulous patient records and coordinate resources for continuity of care.
• Initiate referrals and engage families to ensure a holistic care experience.
