Job summary

Ancora Home Health and Hospice in Anchorage, AK is seeking a compassionate Bridge Care Coordinator to support patients and families at home. This role focuses on coordinating transitions between care services, including hospice, and ensuring peace of mind for patients.

You’ll visit patients, educate families about available services, connect them with community resources, and collaborate with our team to deliver quality, person-centered care.

Qualifications

• Healthcare experience required.
• Strong organizational and computer skills.
• Valid driver's license and clean driving record.
• Reliable transportation and current auto insurance (MVR check required).
• Negative TB test.
• 18+ years old.
