Qualifications

Responsibilities

• Develop expected patient outcomes and coordinate resource use with providers.
• Provide nursing support to optimize patient outcomes and education.
• Coordinate continuity of care with patients and families after hospital events.
• Educate patients and families about medications, procedures, and care plans.
