Qualifications

• Bachelor's degree in nursing or related field required.
• Current RN license in state of practice and BLS certification.
• Approximately 5 years of clinical experience preferred.
• Oncology nursing experience or ability to obtain oncology cert within 6 months.

Responsibilities

• Coordinate comprehensive care across the continuum for patients.
• Collaborate with a multidisciplinary team to ensure timely screening, diagnosis, and treatment.
• Assist patients and families with education, barrier assessment, and support.
• Facilitate discharge planning and referrals; ensure patient/family understands care plan.
• Support end-of-life care and participate in process improvement activities.
