Job summary

Optum is seeking a Case Manager in Boulder, CO to coordinate patient services through an interdisciplinary process and manage discharge planning. This on-site role requires hospital experience, weekend coverage every two weeks, and collaboration with physicians, social workers, and care teams.

Qualifications

• RN or Social Work licensure eligibility required (CO).
• 2+ years of clinical experience in an acute care setting.
• Live in the Boulder, CO region; on-site work only.
• Willing/able to work one weekend shift every two weeks.

Responsibilities

• Coordinate patient services through an interdisciplinary process.
• Assess appropriateness of admission and level of care.
• Plan discharge and ensure timely transfer of patients.
• Educate patients and families about options within care levels.
