Job summary

University of Colorado Anschutz is seeking a Primary Care RN Case Manager for a full-time, 1.0 FTE role based in Denver, CO. This outpatient care coordination position focuses on coordinating patient care across clinic lines, triaging calls, and assisting families with community resources.

Responsibilities include medication administration and nursing duties within a team-driven, Patient-Centered Medical Home environment.

Qualifications

• Bachelor's degree in nursing from an accredited school.
• Current RN licensure in good standing (any state).
• BLS/CPR certification by date of hire.
• Ability to work onsite in Denver.

Responsibilities

• Coordinate patient care across clinic and connect patients with specialty care.
• Triage incoming patient calls and assist families with accessing community resources.
• Provide clinical care for adults and children as part of care coordination and triage.
• Document patient care using the electronic medical records system.
• Participate in care coordination, health coaching, and quality improvement activities.
